Community Outreach Support

In our experience, we know that some service users, who live in their own homes require regular, tailored, and planned one-to-one support sessions to help expand their interests, explore new social opportunities, and build positive experiences and skills.

Community Outreach

For these individuals, support to access the community works best for them, where accommodation-based care and support are not considered appropriate. Here at Reeson Care, we support our service users to develop skills based on individual interests and specific needs, for example:

  • Finding and engaging in new community interests or opportunities

  • Support with the use of public transport and the provision of travel training to enable confidence building, road safety awareness and overcoming the challenges of independent travel.

  • Accessing community-based activities such as attending clubs, sporting activities social-recreational and leisure events or hobbies, such as train, plane spotting, bird watching, horse riding, music, country parks, etc.

  • Organising and accompanying on day trips and holidays

  • Developing or maintaining life skills, such as food shopping,

  • Personal care.

  • Support to become involved in their local community, engaging in social activities and making friends.

  • Supporting people to access college, or voluntary / paid work and providing support in situ
