We are fortunate to be commissioned to provide care and support to people in our communities. We recognise these privileges, for which we remain enormously grateful. Our focus always is to enable our service users’ safety and to improve their quality of life.
Our steadfast commitments and the outcomes we promise, which our service users have consistently achieved through the extensive range of support we offer in all our services include the following:
Risk and Harm Minimisation
We will remain alert to the risks and behaviours that challenge services and deploy our knowledge, skills, and experience appropriately and in a timely manner to keep our service users safe. Strategies include communication, anticipations/de-escalation, planned and reactive interventions, modifications to the environment, behavioural modification through positive reinforcements, etc.
Recovery and Relapse Prevention
By identifying the stressors and triggers, we aim to avoid “revolving door” episodes by working with our service users to stay well and live in their own homes sustainably, enabling and facilitating their access to supportive community networks and resources for resilience to live with their conditions.
Health and Well-being Optimisation
Maintaining a lifestyle that supports longevity, through health education, exercise, healthy meals, accessing healthcare facilities and professionals.
Building on Residual Skills, Knowledge, Confidence, and Abilities
This is the starting point of our strength-based model of care. Recognising our service users’ resources is vital to their recovery, growth, and fulfilment.
Acquisition of Independence and Autonomy
There should be no end to learning. Promoting our service users’ independence and enabling their self-care skills to flourish as much as possible, are at the forefront of our endeavours.
Social Inclusion and Engagement
Our service users will not be left behind or stigmatised. They will be supported to be a positive contributor to the community they live in and participate in their civic duties, enjoy the facilities in the communities of their choice and not be discriminated against.
Safe and Comfortable Environments
A safe home to live in and a restful mind is assured with us.
